Hepatic biotransformation of xenobiotics is carried out by a coordinated set of Phase I and Phase II enzymes, both of which have a highly interactive relationship with the endocrine, nutritional, and excretory functions of the body. These fundamental processes of the liver determine whether or not the drug will be metabolized or not. The number, activity, and intrinsic toxicity of drug-metabolizing enzymes in the liver are influenced by such factors as age, sex, route and rate of drug administration, diet, hormonal and endocrinological status of the host, and the physical form of the xenobiotic. The hepatic biotransformation is divided into two major pathways: Phase I detoxification and Phase II conjugation. The enzymes of Phase I are divided into three subgroups: cytochrome P450 (CYP), flavin-containing monooxygenase (FMO), and alcohol dehydrogenase (ADH). The processes of Phase II metabolism are divided into two subgroups: conjugation and activation. The two major conjugating enzymes, which play a critical role in the detoxification process, are UDP-glucuronosyltransferase (UGT) and sulfotransferase (SULT). In this paper we describe the basic mechanisms of hepatic biotransformation and review the enzymatic alterations in the liver that occur during disease.
